- Dates: ASAP – 3 months
- Schedule: M-F 8a-5p
- Scope: physicals, sick visits, vaccines, health screenings, chronic disease mgmt
- Daily Volume: 18
- EMR: EPIC
- Experience: 2+ years advanced practice required
- License and certification: CA lic, DEA, BLS
- Pay rate: $95 per hour
