NEW Permanent RN Case Manager, Hospice opening near Clinton, WI!
**Requirements:**
- Graduate of NLS School of Nursing.
- Bachelor of Science in Nursing (BSN) preferred.
- Current RN license in Wisconsin and Illinois, or ability to obtain both.
- Valid driver's license and reliable transportation.
**Job Description:**
The RN Case Manager is responsible for providing care to an assigned group of patients and their families, based on the nursing process. This includes assessment, planning, implementation, and evaluation of patient care. The RN Case Manager coordinates services provided by the Interdisciplinary Team to ensure that patient and family needs are met. They ensure that care is provided in accordance with federal and state regulations, Beloit Health System, Beloit Regional Hospice policies and procedures, and hospice standards of practice.
