Job Description: Maxim Government Services (MGS), a division of Maxim Staffing Solutions, is currently seeking Cardiothoracic Advanced Practice Nurse Practitioner (APNP) for the William S. Middleton Memorial Veterans Hospital in Madison, WI. Services shall be provided on the ward and in the clinic setting. Services shall be provided Monday through Friday between 7:30am and 4:00pm.

The Cardiothoracic APNP shall provide the following services which include, but are not limited to:

1. Perform histories and physical examinations on cardiothoracic inpatients and outpatients. The VA staff physician will review and countersign all admitting inpatient, pre-op and consult history, and physical examinations.

2. Provide ongoing inpatient and outpatient care for assigned patients.

3. Prescribe medications within defined scope of practice.

4. Order diagnostic studies such as laboratory tests, x-rays, and electrocardiograms.

5. Carry out health promotion and disease prevention activities.

6. Obtain venous or arterial blood specimens as needed.

7. Initiate and expedite requests for consultations and scheduling special tests and studies.

8. Make daily rounds to observe and record progress notes in the patient’s medical record.

9. Notify the responsible VA staff physician of significant changes in the patient’s medical condition.

10. Dictate discharge summaries. The VA staff physician must countersign the discharge summary.

11. Provide education and counseling of patients and families in preventive care, medical conditions, and use of prescribed treatments and drugs.

12. Perform routine wound care and debridement as needed.

13. Remove chest tubes as directed by the VA staff physician.

14. Remove temporary epicardial pacing wires as directed by the VA staff physician.

15. Order CT/MRI or other non-invasive studies.

16. Order invasive studies as directed by the VA staff physician.

17. 3. The Cardiothoracic APNP shall be authorized to perform the following duties for patients with a life-threatening situation where a VA staff physician is not immediately available.

a. Basic Life Support (BLS) in accordance with current Cardio-Pulmonary Resuscitation standards.

b. Advance Cardiac Life Support (ACLS) in accordance with ACLS protocols, to include defibrillation and cardioversion if the APNP has current documentation of successful ACLS course completion.

QUALIFICATIONS OF PERSONNEL

1. Personnel assigned by the contractor to perform the services covered by this task order shall have a full and unrestricted license in a State, Territory, or Commonwealth of the United States or the District of Columbia. The qualifications of such personnel shall also be subject to review by the VA Chief of Staff and approved by the VA Facility Director. The Contracting Officer will be notified prior to any changes in personnel. VA reserves the right to approve the assignment of individual personnel furnished by the contractor to perform the functions specified in the task order.

2. Must have BLS and ACLS certification to be acceptable.

3. Personnel assigned by the contractor to perform the services covered by this task order shall be proficient in written and spoken English (38 USC 7402).

4. Contractor’s assigned personnel must be computer literate as patient documentation is directly inputted in the patient’s electronic medical record.

5. Any new requirements for mandatory education and/or competency reassessment, which occur during the task order period must be completed by the individual contractor employee(s) within established timeframes.


6. Physical Requirements: The contractor must provide documentation that demonstrates that the all contractor employee’s have a satisfactory health status including a current physical, which includes a current TB test and Hepatitis B vaccination. Proof that a two-step tuberculosis skin testing is up-to-date must be received prior to starting employment. If the provider has had a negative test within the past 12 months, a single step test will be done. In both instances, documentation must be received prior to starting. Contractor must also provide documentation that demonstrates that the contractor employee has a history of chicken pox, varicella vaccine, or a positive immune status for Varicella and has non-restricted physical ability to perform assignments.
