Job Description
Basic Function: Under the direction of primary care physician(s), provide health maintenance as well as care to patients with common acute and chronic illness. The practice is comprehensive, continuous and coordinated. The Advance Practice Nurse is responsible for health assessment, early detection of illnesses, health promotion, and disease prevention. Collaboration and referral to other health care providers, patient advocacy, teaching, and family counseling are major components of the role. The patient age range will vary according to site.
Requirements of this position include:
Certification as a Nurse Practitioner required
Must possess and maintain licensing as required by Illinois State law for APN.
Minimum of one year post-school experience as a mid-level provider preferred.
Valid CPR certification required. ACLS certification preferred.
When requested, obtains/maintains DEA license.
