As a nurse practitioner at Life Line Community (LLCH), you will have thousands of opportunities to provide Medicare patients with the information and tools needed to identify health risks early in order to prevent, delay and alleviate chronic illnesses and disability. Using a community outreach model we accomplish this by giving patients accessibility to preventive health services and health literacy education during day shift hours Monday – Saturday at commonly visited community locations. As the Primary Provider of Services As a nurse practitioner with us, you will be the primary provider of services while most often having support of a medical assistant and access to a collaborating physician as needed. The primary scope of practice includes two non-complex, but vitally important services: the Welcome to Medicare Visit (IPPE) and the Annual Wellness Visit (initial and subsequent). Both services were developed to identify health risks factors early so you may provide patients with personalized preventive recommendations and health education in order to help them improve and maintain their health and well-being. As needed, care coordination and referrals to physicians may be recommended for medically appropriate follow-up. Practice Settings are Out in the Community We have a community outreach model of care offering patients accessibility to quality preventive care and face time with their provider, you. This approach to healthcare will offer a unique reward each day as you see up to 30 new patients at one event and in one town near you. This means as a full-time nurse practitioner you will have the opportunity to provide preventive services to roughly 7,200 Medicare patients in 240+ towns near you. Events are scheduled well in advance, during day shift hours and are not held in homes, on Holidays or on Sundays, which means you will have the flexibility of knowing and planning your schedule. Job Requirements • Full-time nurse practitioner’s autonomously provide preventive services 4-5 days a week during day shift hours to help aging adults prevent, delay and alleviate disease and disability. • Active State License • NPI # • Desire to be in a patient-centered, healthcare setting • Excellent bedside manner with solid customer-oriented presentation skills • Capable of multi-tasking in a fast-paced environment, where you are often moving from station to station • Ability to working autonomously and adapt to changing environments • Talent for cultivating a positive and energetic team atmosphere • Strong computer proficiency • Flexibility to travel by car in company vehicle to community events within 1-4* hours from home location • Must have a valid Driver's License with a clear driving record to drive company car • ANCC or AANP board certified required • Ability to lift up to 50 lbs. on a consistent, daily basis • Actively or recently participated in service to Medicare patients and Medicaid patients (preferred) *Dependent upon employee classification, job location and or patient event schedule.
