Specialty needed: Family Nurse Practitioner Qualifications: Must have a current state license, an active DEA, and about 2 years of relevant experience Location: 30 miles outside of Houston, TX Practice setting: Outpatient Community Health Center Dates or start date: Ongoing Per Diem shifts available starting on 2/1/15 *Future shifts will be available beyond the dates listed above Shift times: Mon/Tues/Thurs/Fri (7:00a.m. - 5:00p.m.) Wed (7:30a.m. - 5:30p.m.) Ages seen: May treat children from birth to 21 yrs of age Patients per day: 20-25 Support staff: This facility is home to a very organized and friendly team of support staff EMR: Yes Procedures: No procedures DEA required: Yes Additional information: Typical cases include sick visits, annual exams, immunizations, etc. We are seeking a provider who is interested in covering future Per Diem shifts.
