Job Details:
Must be available to work 4/10 hour days and every 3rd weekend, with potential for night/weekend call sharing.

Our client boasts five skilled nursing facilities as well as three assisted living facilities.

Performance reviews with opportunities for raises occur at 3, 6 and 12 months, then yearly thereafter.

Nurse Practitioner:

The Nurse Practitioners vision is to provide expert, geriatric-focused clinical care that models high professional standards while individuating care for the older and frail adult in the post acute care, long term care, and assisted living settings; and to link provider to provider communication between acute care, post acute care, and community care settings.

Responsibilities of the NP includes, but is not limited to:
• Rounds on post acute care patients providing clinical care from admission to discharge
• Provides bedside medical visits on long term care patients for both primary care and acute illnesses
• Visits assisted living patients as needed for minor acute illnesses
• Accurately documents in the electronic medical record daily
• Accurately completes visit and billing log daily
• Communicates with nursing staff, inter-disciplinary team, physicians and families as needed to promote optimum care of each patient
• Attends monthly NP meetings
• Pursues professional responsibility and growth, clinical knowledge and EBP, positive facility staff relationships, communication with community and specialist providers.

The Nurse Practitioner must possess an advanced practice nursing degree from an accredited college or university and possess a clear unencumbered license from the State of Colorado as a Registered Nurse and Nurse Practitioner.

Prefer a master's prepared and national certification as either an adult/geriatric nurse practitioner or acute care nurse practitioner. Prefer at least 2 years of NP practice experience in either ICU/acute care or urgent care experience.

Must have active prescriptive authority with the Colorado Board of Nursing and active DEA license.

Prefer at least one year of experience working with adult and geriatric patients in subacute or long term care setting.
