Primary Care Nurse Practitioner - (NP-Primary Care) Locum Tenens

New Locum Tenen opportunity for Nurse Practitioners in the Modesto, California area for home health assessments beginning in October and will extend through Jan. 1. The provider will be identifying, evaluating, and addressing disease prevention and health promotion issues of the Medicare Advantage member while conducting health assessments in medical facilities and member's home. The provider will do a review of systems, history, physical exam as well as any indicated screenings which may include, bone density, urinalysis and blood draws. A primary care nurse practitioner (locum tenens) diagnoses and treat patients, order diagnostics, conduct physical exams, interpret x-rays and other tests, and also educate patients on maintaining proper health. In addition to a nursing license, a primary care nurse practitioner holds a certification as a nurse practitioner, and typically a master's degree. A primary care nurse practitioner works with physicians on duty to help evaluate and treat patients.

Required Qualifications
Current and active state license is required.
Current and active driver's license is required.
Client requires at least 1 year of experience in geriatric patient care or clinic involvement with geriatric population.
Client requires at least 1 year of experience in providing home health care visits (preferably in geriatric patient populations).
Client requires at least 1 year of experience performing physical examinations, performing diagnostic tests and screenings, developing treatment plans.
Client requires at least 1 year of recent experience in phlebotomy.
Experience in gerontology and the ability to work with little or no direct supervision in the performance of these clinical duties are critical factors.

Preferred Qualifications
The provider identifies, evaluates, and addresses disease prevention and health promotion issues of the Medicare Advantage member while conducting health assessments in medical facilities and member's home. The provider will do a review of systems, history, physical exam as well as any indicated screenings which may include spirometry, bone density, urinalysis and blood draws. This position also requires that recommendations of care to the member. The practitioner will be responsible for delivering or mailing (where applicable) collected specimens to lab drop off locations, complete and accurate documentation of all present and observed diagnoses for each member as well as educating patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications. Some travel may be required.
