Job Details:

Description: The Care Manager is a key member of the VQCP Clinically Integrated (CI) team. This individual will be responsible for coaching, educating and empowering patients and their family/support systems to achieve better health at a lower cost, under the direction of the patient’s physician(s).
Key Responsibilities:
• Conduct comprehensive patient assessments to include: psychosocial, functional, and chronic condition knowledge and understanding to identify gaps in care and barriers to optimal care
• Develop care plans, in conjunction with physician and patient, to address and achieve immediate and ongoing needs and goals
• Coach and support patients utilizing motivational interviewing and behavior change techniques
• Facilitate patient care coordination by working with patients, providers, health systems, payers, family/support systems and community resources
• Advocate and resource for Patient/Family/Support system
• Maintain knowledge of disease processes and treatment options
• Consistently and thoroughly document activities and interactions
