Job Description
Education: • Graduate from a Licensed Nursing Assistant Program. Experience: • Prefer a minimum of one year of clinical experience. Licenses/Certifications: • Must maintain and show bi-annual proof of an active state nursing licensure and place a copy of the license in the personnel file maintained by the Program Manager. • Must maintain and show annual proof of active CPR certification. Job Summary: In a team relationship with the Registered Nurse, Licensed Practical Nurse, the LNA, contributes to the nursing process through on-going systematic data collection, recording observations, goal identification, and patient health education. Undergo training and have it signed off by Nurse Manager/Clinical Coordinator or designee on intake screenings focusing on medical, mental health and dental. Essential Functions: • Responds to medical emergencies throughout the facility under the direction of professional staff. This includes the ability to perform continuous CPR until properly relieved. • In a team relationship with the RN supervisor, contributes to the Nursing Assessment. • Initiates the admission process and assists with identification of recurrent health problems, symptoms and behavioral changes of the patient. • Collects pertinent data through observation, interview, other members of the health team, and communicates changes in the patient condition to other patient care providers. • Records all data obtained through assessment in the patient’s health record. • Forwards all intake screenings to LPN/RN for review and sign-off. • Teaches health care information to patients and correctional staff. • Adheres to safety and security policies and participates in disaster drills. • Adheres to Universal Precautions and other appropriate infection control practices. • Respond to “code” or patient crisis as set forth by the sites policy and procedure.
