Shall be a graduate of a nationally accredited associate or bachelor degree program in nursing and hold current license as a registered nurse (RN). 1.2. Must have a least 24 months experience within the last 48 months working as a RN. 1.3. Shall have certification in Basic Cardiac Life Support (BLS). Advanced Cardiac Life Support (ACLS) certifications must be obtained through the American Heart Association or a Red Cross-approved training program. 2. SPECIFIC TASKS: Duties include but are not limited to the following: 2.1. Provides coordination of internal and external facility patient admissions and transfers through a centralized transfer center. Provides oversight of capabilities available and capacity of bed status at MTF. Communicates with multiple providers across all levels and specialties of care. Provides feedback on process to assist in managing and improving the Transfer Center Program. 2.2. Accepts all calls regarding internal or external transfers or admission of patients. 2.3. Ensures efficient and smooth processing of transfers and admissions. 2.4. Checks eligibility by verifying patient is registered in CHCS and DEERs. Updates demographic information as needed. Obtains current and accurate contact information. Maintains familiarity with any applicable Veteran’s Administrations Systems or contacts needed to verify eligibility. 2.5. Documents all medical information given by facility appropriately. 2.6. Provides nursing consultant on specialty/surgical care needed in accordance with patient’s diagnosis. Ensures clarity in entry point for patient (ED or appropriate ward). 2.7. Remains continually knowledgeable of MTF capabilities and capacity to expedite outcome of transfer and accepting specialty/ward. 2.8. Facilities quick coordination with appropriate charge nurse on ward to verify bed availability for every transfer or admission that is within capability. 2.9. Facilitates appropriate doctor-to-doctor calls. Ensures sending facility is aware the doctor must call to give current report. 3.0. Coordinates with sending facilities to ensure all appropriate medical records are available for review upon patient arrival to facility. 3.1. Remains current on clinical skills to expedite patient care and to assist nursing staff on the floors with all transfer patients if needed. 3.2. Establishes working relationship with Admissions staff to ensure appropriate admissions occur in a timely fashion. 3.3. Shall participate in activities to the extent required by the MTF QA plan or regulation. The government will evaluate the RNs professional performance under this contract using quality assurance standards specified in AFPD 44-1, Medical Operations. 3.4. Collaborates with MTF departments for program planning, development and evaluation in support of the MTF Strategic Plan/Mission Support Plan. Develops interdisciplinary management style through effective communication and collaboration with appropriate departments. 3.5. Delegates tasks to appropriate level and directs support staff in meeting MTF operational requirements.   3.6. Facilitates process improvement to enhance the delivery of nursing care. 3.7. Conducts inspections as directed by requesting office/regulatory body. 3.8. Provides for continued professional growth of assigned staff. 3.9. Leadership and/or participation in committee and functions associated with the facility that might include but are not limited to; Nursing Practice, Infection Control, Education and Training and similar. 3.10. Participates in professional development activities and maintains professional affiliations. 3.11. Attends conferences and workshops related to duties. 3.12. Maintains current knowledge of disease management, preventive screening standards/requirements, and current trends in population-based health management. 3.13. Maintains current knowledge of practice standards and facilities compliance. 3.14. Discusses, supports and advocates ethical decision making for staff and patients. 3.15. Champions change by challenging status quo; acts as catalyst when appropriate and manages implementation effectively. 3.16. Provides input to educate caregivers and patients on prevention/disease management programs. 3.17. Effectively utilizes information systems to retrieve, analyze, update, and record nursing data. 3.18. Completes reports to provide current and meaningful data from which assessments can be made to determine effectiveness of prevention efforts. 3.19. Meets all suspense dates. 3.20. Prioritizes requirements and makes timely and appropriate decisions impacting nursing care. 3.21. Accesses information systems to analyze, track and quantify program effectiveness, and to manage the dissemination of information critical to the success of the population health management. 3.22. Forwards questions regarding medical necessity, appropriateness of care, and quality concerns to PCM providers sing established guidelines. 3.23. Intervenes with appropriate individuals/departments regarding delays in service that may impact quality patient care. 3.24. Demonstrates strong customer service skills and fosters development of those skills throughout entire MTF. 3.25. Communicates and collaborates with the network facilities and appropriate members of the health care team to ensure that needs of assigned patients are met without duplication of efforts. 3.26. Coordinates and participates in interdisciplinary team meetings, designated facility meetings and Care Coordination meetings. Shares knowledge and experiences gained from own clinical nursing practice and education relevant to nursing. 3.27. Participates in the orientation and training of other staff. May serve on committees, work groups, and task forces. 3.28. Must maintain a level of productivity and quality consistent with complexity of the assignment; facility policies and guidelines; established principles, ethics and standards of practice of professional nursing. 3.29. Completes medical record documentation designated tracking logs and data reporting as required by local MTF, Air Force (AF), and Department of Defense (DoD) instructions, policies and guidelines. 3.30. Completes all required electronic medical record training, MTF-specific orientation and training programs and AF and DoD-mandated nurse specific training. 3.31. Enters patients into all variations required EMR to include but not limited to CHCS/ESSENTRIS.
