Job Description
Luke and Associates has an opening for a Nurse Case Manager at Travis AFB, CA.
Assess patients for case management from a variety of sources such as discharge/disposition planning and referrals.
Develop appropriate patient specific plan of care to include short and long term goals.

Job Experience:
Minimum of 3 year’s experience for ADN graduate or 2 years for BSN graduate working as a full-time RN Case Manager
Job Education:
Graduate of accredited registered nursing program.
Job Certifications:
Certified Case Manager by CCM or ANCC or eligible w/ 3 years exp.for & or 2 years for BSN in full time Case Management role. BLS
Job Licensing:
Active and current, valid, unrestricted license to practice nursing as a registered nurse in any US State/jurisdiction.
