Job Description
The LPNs are responsible for delivering in-home health services and patient monitoring as directed by our medical protocols and physician and advance practice provider partners. Our LPN's are provided with specialized training, equipment and supplies required to respond to urgent & emergent situations and are expected to approach each patient encounter with a high degree of empathy and compassion.
• In-home assessments, screenings, and on-going home visits under physician direction
• Conducting and documenting medical history, medication inventory and physical exam
• Conducting and documenting home safety inspections
• Administering medication (incl. injections / vaccinations)
• Appropriate wound care
• Point of care testing and phlebotomy for commercial lab testing
• Starting IV’s and administering IV medications
• Porta-Cath and PICC-line care
• Foley catheter and PEG care
Required Qualifications:
• LPN Certification with prior experience as an LPN in direct patient care role
• Valid driver’s license with acceptable driving record
• State required BLS, and ACLS certifications must be current
Target Compensation Guidance:
$50,430 - $74,444 a year depending on experience level
LPNs will operate on a 12-hr shift schedule
