Position Title: Intake Coordinator

Your Role with the Company:
- Provides support and educates patients in care along with family about palliative care
- Ensures accuracy and entry of medical history into electronic medical record

- Facilitates patient’s virtual visits with Provider
- Monday - Friday
Background Profile:
- Strong interpersonal communication and management skills
- 2 years or more of experience ideal
- Valid U.S. driver’s license
