LPN - LONG TERM CARE

HumanEdge is looking to hire a Licensed Practical Nurse to work in an Assisted Living facility in Dutchess County, NY

8-hour shifts, 7:00am-3:00pm, 3:00pm-11:00pm

• Follow a designated care plan in accordance with patients’ needs
• Perform assigned duties. Including administration of medication, wound care, treatments, and procedures
• Monitor client’s conditions; reporting changes to RN’s
• Accurately document observations, interventions and evaluations pertaining to client care management and services provided
• Typical daily procedures include IV fluids, IV meds, foley catheters, gastrostomy tubes, Wound VACs, oxygen management, BIPAP/CPAP, fingerstick blood sugars, phlebotomy

• 5 shifts per week includes every other weekend
• BLS
• State License
• NY requires an annual check completed 30 days prior to start.
• 1 year of geriatric or MS experience
• Charting system Matrix Care
• Education: Graduate of a State approved and accredited School of Nursing.
• Experience: 1 – 2 years’ experience in Nursing Home or Hospital setting
• Other: Demonstrates initiative, judgement and dependability.
• Good verbal and written skills.
