Position Overview
As a Memory Care Director LPN/RN, you will implement and execute programs that ensure the highest level of care for our residents with dementia while serving as the primary liaison between residents, families and staff. This is a hands-on role where, above all, you will exhibit a passion for serving seniors while contributing to the positive environment that our 100+ residents are proud to call home.

The Memory Care Director is responsible for developing and executing a best in class approach to Memory Care, utilizing all available components to maximize quality Resident care. You will oversee the day to day operation of the unit and are responsible for recruiting, training, coaching, counseling and directing the staff in the Memory Care setting in accordance with company policy and procedures as well as current federal, state, and local regulations and standards. You will establish and implement policies, procedures and practices, develop staffing plans, reports and budgets, develops the functional schedule, oversee Individual service plans, monitor documentation compliance and coordinate the functional schedule and activity plan.

You will closely collaborate with other departments, outside agencies, families and physicians to ensure coordinated Resident care delivery. The ultimate goal of quality Resident care is to enable each Resident, where possible, to return to the highest level of functioning, to attain a new stage of independence, or to at least prevent regression. Resident care is not passive maintenance; an active approach will always be taken.

Here is what we’ll expect from you:

  • LPN or Geriatric or Dementia Activity Specialist
  • Combination of education and/or experience equivalent to graduation from an accredited college or university, with major work in related field.
  • Licensed Nursing Home Administrator Preferred
  • CPR
  • Commitment to support the mission, vision, and goals of the organization, upholding and promoting company culture and vision.
  • Knowledge about the disease process, the changes associated with dementia illnesses, how they affect the residents’ ability to function, and the adaptive strategies that help maintain residents’ abilities
  • Degree or equivalent experience and certifications in the areas of social services, nursing, behavioral health, etc.
  • Coordinate memory care admissions including supervising move- ins to be sure accommodations are as desired, assessments are completed, and care plan is immediately implemented
  • Plan, direct, and evaluate the activities of the department in compliance with all applicable rules, regulations, and guidelines of care as well as policy, procedures and practices. Assure activities are effectively planned on a functional schedule and carried out by staff in appropriate manner. Promote the philosophy that everything is an activity.
  • Partner with leadership team for the establishment and maintenance of a medication system which adheres to state, federal and company laws, regulations and guidelines
  • Conduct individual resident history, preferences assessments, and develop individualized service and program plans
  • Administer Employee Policies and Procedures. Develop, maintain and periodically update written policies and procedures and job descriptions. Orient staff as required, train on policies and procedures and frequently updated Best Practices.
  • Schedule, manage and supervise memory care staff by providing assignments, support, direction, feedback, and training to ensure safety and appropriate level of care. Conduct regular evaluations as required/needed.
  • Develop departmental budget and staffing plan.
  • Make reports and recommendations as needed/ required to the Executive Director and to the Administrator concerning the operation of the Memory Care setting.
  • Develop, implement, and maintain an ongoing Quality Assurance program and provide appropriate plans of action to correct any identified deficiencies utilizing quality enhancement tools and family feedback. Maintain an active and effective fall prevention and hydration program.
  • Take initiative to actively plan, develop, and implement expansion and/or innovation in program services. Update staff in regards to new program requirements or initiatives.
  • Coordinate care planning with families, physicians, and home health agencies while working within the Communities’ policies and procedures
  • Actively communicate and work with Families and Residents of Memory Care to update on issues and when appropriate to assist in safe, respectful and appropriate transitions to other care settings, as necessary.
  • Develop methods for coordination of the Memory Care department’s services with other Resident services, to ensure the continuity of the Residents' total regimen of care.
  • Oversee surveys and inspections made by government agencies as well as internal QA or regulatory or risk audits.
  • Complete appropriate forms, reports, evaluations, and studies. Monitor and maintain documentation compliance in all areas to include MAR’s, incidents, Resident clinical information, QA, Safety and required reports.
  • Assist in Resident care on a regular basis. Model behavior and interventions with staff.
  • Develop and attend in-service classes, Resident support meetings and staff meetings.
  • Orient new employees. Lead and coach certified nursing assistants to improve performance. Provide other departments with input and guidance on our best in class approach to care.
  • Obtain and demonstrate knowledge of the Discover Your Spark Lifestyle and Wellness philosophy and its inherent core values. Support and participate in Discover Your Spark (e.g. assist with feedback groups, assist in new initiatives, encourage Residents and act as champions of fun).
  • Perform other duties as assigned and as outlined in policies and procedures and disaster and fire plan.
  • Facilitate a pro-active problem-solving approach to challenging resident behaviors.
