[Register to View] msonormal">This will be a 55-bed hospital-based long term care facility located adjacent to affiliated hospital. The nursing home is certified in the Medicare and Medicaid programs. Families, volunteers, and the community are very active in our facility. They use Matrix EMR system. Entire nursing home is located on one floor.

[Register to View] msonormal">Nursing: The licensed nursing staff consists of registered nurses (RN’s) and licensed practical nurses (LPN’s). Licensed nurses are on duty 24 hours per day. Services include pain management, skin care and pressure sore prevention, fall prevention, feeding tubes, indwelling catheters, medication management, and other associated nursing care.

[Register to View] msonormal">The desired staffing ratios are 1:18 (dayshift) and 1:27 (nightshift).

[Register to View] msonormal">LPN Job Duties to include the following:

[Register to View] msonormal">•Identifies patient care requirements by establishing personal rapport with potential and actual patients and other persons in a position to understand care requirements.

[Register to View] msonormal">•Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions.

[Register to View] msonormal">•Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations.

[Register to View] msonormal">•Resolves patient problems and needs by utilizing multidisciplinary team strategies.

[Register to View] msonormal">•Maintains safe and clean working environment by complying with procedures, rules, and regulations; calling for assistance from health care support personnel.
