LPN licensed practical nurse Home Health Center City Philadelphia and NE, Delaware County, Chester County, Montgomery County & Bucks County
A licensed practical nurse assists in the provision of nursing care to the home health patients.
Collaborates with the patient's case manager and other member of the patient care team.
Independently makes and documents home care nursing visits.
General Responsibilities and Accountability
1 Provides home care nursing visits assigned.
2 Submits completed documentation of the home care visit within the established time parameters.
3 Participates in the quality improvement and utilization review activities of CHC.
4 Attends multidisciplinary patient care conferences as requested.
5 Attends staff meetings and in-services.
6 Instructs patients and caregivers according to the plan of care.
7 Makes nursing assessments and reports significant changes in the patient's condition.
8 Recommends changes to the plan of care based upon assessment findings.
9 Demonstrates a high degree of commitment to customer service and quality care provision.
10 Exhibits good written and verbal communication skills.
11 Provides care according to the organization's policies and procedures.
12 Refers patients to community resources as appropriate.
13 Maintains accurate and current drug profiles for patients assigned.
14 Completes other duties as assigned or requested.
Education and Licensure
Licensed as a practical nurse in the state of PA. Possesses two (2) years experience as a
Licensed Practical Nurse.
Competency and Skill Requirements
Excellent written and verbal communication skills. Able to utilize the telephone for internal and
external communications. Knowledgeable of nursing assessment skills. Competent in the provision
of nursing care. Reports to case manager.
