Job Description
LPN – Continuous Care (+ Extended Hours & Hope Kids Care Respite)
Provide one-on-one care to Continuous Care hospice patient or Hope Kids Care patient (child with life limiting illnesses for respite care) in their home or facility for a 12 hour shift. Computer skills and minimum 1 year experience required. Three 12 hour shifts per week. Total = 72 hours Bi-weekly - Rotating schedule - Mon-Sun - includes 2 weekends a month. Scheduled shift start and end times may change if working Hope Kids Care Respite.
#2024 - Nights – 8pm to 8am - Patients located in Lee, Hendry and Glades Counties
