QUALIFICATIONS:

Licensed Practical Nurse must be currently licensed and in good standing with the State Board of Nursing. The LPN shall have at least 2 years experience in nursing as an LPN and at least a minimum of one-year experience in hospice or long-term care. The LPN must also have good communication skills both oral and written. Desire to learn and demonstration of aptitude for the hospice philosophy and palliative care is required.

RESPONSIBILITIES:

The LPN is responsible for the management of care for assigned patients under the supervision of a RN using an Interdisciplinary Team approach. The LPN will perform duties according to the Hospice policy and procedure and the scope of practice as defined by the respective State Board of Nursing.

The Licensed Practical Nurse works in collaboration with the DCS/Executive Director the RN Care Manager and the Interdisciplinary Team to:

  • Assist with planning for client care including admission, discharge and transfer within scope of practice.
  • Perform routine or on call home visits as assigned by DCS/ED and/or RN Care Manager
  • Provide hands-on clinical nursing, within the LPN scope of practice, which includes interacting with patients, family, Interdisciplinary Team members and other health care professionals involved in the patient’s care.
  • Observe, record, and report the condition of the hospice patient to the LPN’s immediate supervisor for further assessment and intervention, including signs and symptoms which may be indicative of change in the patient’s condition.
  • Implement interventions as outlined in the Plan of Care to improve uncomfortable situations such as pain, bowel and skin/wound management per Hospice protocol; as well as, other activities for which the LPN has been prepared through basic education and those additional skills which are obtained through approved continued education programs.
  • Assist with Ostomy care and urethral catheterization as ordered.Provide personal care when needed such as bathing, shaving, feeding and massage.
  • Assist in facilitating freedom of choice, relief of suffering and death with dignity for patients facing end of life issues.
  • Assist with planning, implementation, evaluation and recording of client teaching and outcomes of the Plan of Care.
  • Assist in ensuring services provided are in accordance with the patient’s Plan of Care.
  • Assist in ensuring the Plan of Care involves the active participation of the patient, family, Primary Care Physician and the Interdisciplinary Team.
  • Educate patients and families on the disease process and expectations related to the progression of the disease.
  • Intervene in situations involving unsafe or inadequate care.
  • Evaluate patient responses to therapeutic interventions, identifying and reporting and adverse / undesirable reactions.
  • Document and report all pertinent activities and interventions on the appropriate records.
  • Participate in Interdisciplinary Team meetings and assist in updating the Plan of Care for assigned patients based on identified problems and needs, as appropriate.
  • Assist with QAPI activities.

Benchmarks

  • Ensure cost effective practices by self and others.
  • Provide care to manage patient symptoms.
  • Utilize the medication formulary whenever possible for the management of patient symptoms.
  • Reports to the RN Care Manager, MD and/or DCS/ED as patient condition changes.

PROFESSIONALISM:

  • Work as a team member at all times and uphold the Hospice Code of Ethics.
  • Ensures a patient centered culture through CARE (Compassion, Advocate, Respect, and Educate).
  • Work as a team member to fulfill all aspects of Our Gold Standards on a daily basis.
  • Providing a positive image of the Hospice at all times.
  • Adheres to all HIPPA, HITECH, EPHI and company policies and procedures regarding patient protected information.

ORIENTATION AND TRAINING:

Hospice will provide orientation and training, as necessary, to assist the Licensed Practical Nurse in performing the responsibilities listed above. Training’s may be in-house or outside seminars. Evidence of orientation and training will become part of the personnel file.

ON CALL:

The Licensed Practical Nurse may take call according to a rotating schedule. During the hours when on call, the Licensed Practical Nurse is required to follow the On-Call Protocol and responds to all calls and messages within 15 minutes. The LPN will have a designated RN as back up at all times. The backup RN is to be notified of changes in patient conditions requiring changes in interventions and/or death.

WORK CONDITIONS:

May require sitting or standing for prolonged periods of time. Driving/travel required. May require lifting, pushing or pulling up to 50 lbs.

Job requires the ability to:

  • Access patients’ residence and all rooms therein
  • Reliable transportation
  • Communicate effectively with patient/families both verbally and written
  • Communicate the mission and vision of the hospice

ACCOUNTABILITY:

LPNs will work under the direct supervision of the ED/DCS, or RN designee, at all times.
