Job Description

We are working with a very reputable management organization seeking an Advance Practitioner to assist patients with chronic and complex medical conditions in the Assisted Living Home in Virginia Beach, VA. In conjunction with the collaborative physician, the Nurse Practitioner will diagnose, manage, and assist in preventing acute and chronic illness and disease while promoting wellness. Advanced Nurse Practitioner

  • Monday to Friday; 8:00am-5:00pm or 4- 10hour shifts
  • Coverage dates: January 6th for up to 6 weeks
  • Will consider permanent and/or locums to perm
  • Performing in-depth health risk assessments on all assigned plan members
  • Reviewing and interpreting the medical, surgical and social history of members
  • Performing appropriate screening procedures for members
  • Performing HRA's and H&P’s (History and Physicals) at the Assisted Living Facility
  • Company laptop provided to completed HRA’s
  • Paid Training and Orientation
  • $65 per hour

Requirements:
  • Graduate of an accredited Family or Adult/Gerontological Nurse Practitioner program
  • License: AANP or ANCCC
  • Current Virginia State RN and NP licensure
  • Current CPR certification

Preferences:
  • Long-Term Care experience
  • Previous work with vulnerable adult or geriatric populations
  • Experience with low income and disadvantaged populations
  • Two years of clinical experience as a Nurse Practitioner in an Adult, Family or Geriatric setting
  • Perform comprehensive physical exams
  • Knowledge of community resources
  • Identify, diagnose and document chronic disease in adults
  • Recognize conditions, characteristics, and behaviors that place seniors at risk
  • Pursue feedback, continuing education and professional development
  • Effectively reconcile medications
  • Identify and document the needs of clinically complex members
