Overview
POSITION SUMMARY: The Advanced Practice Registered Nurse (APRN) Virtual Care is a member of a collaborative team who is responsible for providing virtual care to a diverse population of patients across the Ministry through technology in their home environment. The APRN will perform a variety of functions including assessment, diagnosis, and intervention within their scope of practice. Utilizes evidenced based care and clinical decision support to guide practice. Performs ongoing communication and works to promote quality of care through collaboration with physicians, advanced practice providers, and other care team members, patients and families. The APRN demonstrates service excellence and accountability through recognizing the direct relationship between cost-effective and efficient provision of care and services including the impact of access to care.
Call is required with roll.
Qualifications
REQUIRED QUALIFICATIONS:Education:Masters in NursingLicenses/Certifications: Current state APRN license Current state Advanced Practice Provider controlled substance licenseControlled substance registration certificateNational Certification within 3 years of hireExperience: 3 years of clinical experiencePREFERRED QUALIFICATIONS:Experience:FNPWorking knowledge of Epic EMR SystemOther requirements/information: Excellent communication skills, including telephone etiquette and customer serviceUnderstanding of the principles of performance improvement, case management, discharge planning, utilization review, and medical recordsUnderstands the basic workings of Joint Commission, DRG's, and Medicare, Medicaid, and managed care reimbursementDemonstrated ability to work independentlyProficiency with Windows based computer operationsProficient in utilizing an electronic EMR system
