Schedule: flexible 2-3 days per week

Seeking a motivated Nurse Practitioner to join our medical practice part time in Gurnee, IL. Our clinic is in Gurnee, IL- approximately 40 miles (55 mins.) north of Chicago.

We are looking for a compassionate provider who enjoys working with a multidisciplinary team helping others, someone who is looking for extra hours or even a possible equity position in the future. We are looking for enthusiastic and caring providers, those who are dependable, have excellent customer service skills and are looking for a long-term commitment. We are a physical medicine office, and the provider will be performing exams, treatments such as joint/trigger point injections, and assisting with patient care. We will train the right candidate if needed.

Our integrated physical medicine practice takes a multidisciplinary approach to our patient’s pain, health, and wellness goals, using chiropractic, physical therapy, and medical services to help our patients get the most effective results in the shortest amount of time. Our qualified and highly respected medical professionals offer a wide variety of functional medicine services including PT, physical and regen medicine, and medical weight loss. Duties:

• Patient exam/evaluation (medical history, physical exam, review pertinent records/labs/reports)
• Order, draw and review labs
• Diagnosis and treatment plan formulation
• Patient education to the diagnosis, lab results, treatment care plan options, counseling on health-related topics, prevention, and lifestyle modifications
• Perform Procedures (joint and trigger point injections) as medically indicated
• Management of Patient Care
• Collaboration with the multidisciplinary team for comprehensive patient care
• Documentation and progress notes- timely and accurate

Requirements:

• Completion of a Nurse Practitioner program (NP)
• NP licensed in IL (& WI preferably)
• Knowledge of anti-aging therapy, hormones, weight loss programs and preventative health care programs a major plus but not required

Compensation (range):

• $50/hr $55/hr
