- Successful group hiring - Coastal Tampa area
- Location: Palm Harbor, Florida
- Outpatient Primary Care
- Primarily Quality Care / Managed Care
- Certified Nurse Practitioner
- In Office - True Outpatient only position
- Monday through Friday 8:00am - 5:00pm
- No weekends
- Low patient panel
- Great Work/Life balance
- Detect and manage chronic diseases
- Develop patient strategy to reduce hospitalization and ER visits
- Collaborate with Case Managers and Specialty Providers
- Educate patients on the value of compliance
