Job Description

We are seeking like-minded clinicians to join us on our journey as a Family Nurse Practitioner. In this role, you will be responsible for managing outpatient behavioral health patient care and treatment in collaboration with other team members. Your primary focus will be on providing exceptional patient care, performing health assessments, and actively managing common and complex conditions. You will also be responsible for ordering and interpreting diagnostic tests, formulating care plans, and explaining medical procedures and plans of care to patients and their families. Additionally, prescribing medication or other forms of treatment will be part of your responsibilities.


· Provide exceptional patient care, ensuring a high standard of medical service

· Perform health assessments and screen for preventative health measures

· Actively manage common and complex conditions, adapting treatment plans as necessary

· Order, interpret, and evaluate diagnostic tests to identify and assess patients' clinical problems and healthcare needs

· Formulate and document care plans based on thorough assessments and medical knowledge

· Explain medical procedures and plans of care to patients and their families, ensuring they understand the treatment process

· Prescribe medication or other forms of treatment according to patient needs and medical guidelines

· Participate in an on-call rotation as needed to provide continuous patient care

· Communicate assessment findings and collaborate with the healthcare team to identify gaps in care and improve patient outcomes


· 2+ years of medically-relevant experience preferred, however new graduates are welcome to apply

· Advanced Practice License in Connecticut (or eligibility to apply)

· DEA license (or eligibility for registration)

· Master's Degree in Nursing

· Current Nurse Practitioner or Physician Assistant certification in specialty area (ANCC/AANP)

· Experience with the geriatric population preferred

· Experience with Electronic Medical Record (EMR) systems; eClinical Works preferred

· Demonstrated interest and passion for value-based care delivery models

· Strong oral and written communication skills to effectively interact with patients, families, and the healthcare team

· Bilingual in Italian or Portuguese would be nice, but not required
