Location: Toledo, OH
Overview:
Our team is seeking a Nurse Practitioner (NP) to provide high-quality primary care services to patients within skilled nursing facilities. This role offers the opportunity to work in a collaborative healthcare environment focused on patient-centered care and clinical excellence.
Position Offers:
- Full-time or part-time opportunities available
- Monday–Friday schedule, 8:00 AM–5:00 PM
- Competitive compensation:
- $115,000–$130,000 annually (FT)
- $78,000–$96,000 annually (PT)

Key Responsibilities:
- Provide primary care services to patients in skilled nursing facilities
- Assess, diagnose, and manage acute and chronic medical conditions
- Develop and implement patient treatment plans
- Collaborate with interdisciplinary healthcare teams to coordinate care
- Maintain accurate and timely clinical documentation
- Monitor patient progress and adjust care plans as needed
- Educate patients and families on treatment plans and healthcare needs
Qualifications:
- Active Nurse Practitioner (NP) license
- Minimum of 2 years of NP experience
- Experience in primary care, geriatrics, or skilled nursing preferred
- Strong clinical assessment and communication skills
- Ability to work independently and collaboratively within a healthcare team
