Alaska - Family/Urgent Care/ER
Arizona – Urgent Care with some Occupational Medicine - multiple locations – DOT required
Arizona – ER – Yuma
Minnesota - ER
New Mexico – Urgent Care and Emergency Medicine – ANY STATE LICENSE – Indian Health
North Carolina – Urgent Care
North Dakota – Emergency Medicine South Dakota - ER – ANY STATE LICENSE *blocks of 14 shifts/month – Indian Health
Utah – Provo – Urgent Care with Occupational Medicine – DOT Required
