High value assessment and care management group seeks a Nurse Practitioners/NPs for per diem performing in-home member assessments!

About the Preventive/Quality Assessments position:
- Home assessments take 1 hour to complete with documentation to be submitted within 24 hours of visit
- At least one day of availability during the week ideal. Provider will review past medical history in EMR, check complications that may have occurred since last visit with PCP, medication review/reconciliation, history and physical, head to toe exam
- Coverage/payment for no shows
- Tablet provided with internet access for documentation as well as medical supply bag
- Ideal candidate has experience in Primary Care/Internal Med, experience with diagnosis and assessment of chronic illness, must be able to establish relationships, be personable, flexible and adaptable
Requirements:
- Current NP and NPI Licensure in your state of employment
- 1 full day of availability during the week
- Open to licensed new graduates!
- MSN/ APRN (Advanced Practice Registered Nurse)/ARNP / FNP, GNP, ANP, ACNP, NP-BC, Family, Adult, Acute Care or Gerontology Nurse Practitioner or will conduct exams and tests for patients ages 50 and up
Compensation:
- $100/per assessment
- Mileage reimbursement
