Job Description
- Full-time Nurse Practitioner for Care at Home program
- Comfortable with visiting patients in their homes
- Comprehensive care to elderly and managing care
- Experience in house calls preferred for mid-levels
- See between 6-12 patients per day
- Knowledge of EMR (Allscripts) and documentation a plus
- Conduct face-to-face medical visits
- Complete comprehensive health assessments
- Order appropriate lab tests and diagnostics
- Timely and accurate documentation
- Communicate directly with PCP's and Specialists
- Participate in On-Call programs for evenings/weekends
