Job Description
Judge Healthcare is actively seeking a part-time Nurse Practitioner for Home Assessments with a Healthcare system based in the Boulder County, CO area.
Terms: Part-time, Flexible weekly schedule!
This job will have the following responsibilities:
- Conduct in-home assessments on health plan members
- Check vitals and conduct physical exams
- Identify needs and opportunities to generate clinical referrals
- Identify diagnoses to be used in care management
- Educate members on various topics
Qualifications & Requirements:
- NP license and national certification (AANP or ANCC) required
- Minimum of 1 year clinical experience required, home health care or home visit experience is a plus
- Family, Geriatrics, or Adult certification required
- Excellent administrative, organizational and verbal skills
- Reliable transportation in order to complete home visit assessments
