Full Time nurse practitioner NP position clinical services manager. Great Compensation plan!

Oversight to a team of nurse practitioners and physician assistants who provides health care to patients who are enrolled in the Chronic Care Program to stem hospitalization. This role works in collaboration with VP and Medical Director.

Job Responsibilities

• Leads Multidisciplinary team meetings with advanced practice providers, registered nurses, social workers, community health workers and other members
• Complete comprehensive history and physical examination on patients in an in-home visit when needed for team.
• Participates in interviewing, hiring and training.
• Collaborates with the clinical team in the management of selected high-risk patients

• Audits clinical documentation and provides feedback to clinical colleagues
• Leads Multidisciplinary team meetings.

Requirements

• Holds active, unencumbered license in state of practice
• COVID vaccinated
• Credentialed in adult, family, or geriatric care
• Has active prescriptive authority
• Nurse Practitioner with at least 2 years experience in care of adult, chronically ill patients
• 2 years of management experience and leadership skills
• Driver’s license without restrictions and good driving record
• Working knowledge of computers and ability to document effectively and efficiently using an EMR system
