Job Description
- Full-Time W2 salaried position with full benefits
- Provide state of the art care to Medicare patients in an outpatient setting
- Bilingual (English and Spanish) required
- Interest / Experience in Geriatric MediCare population
- See 8-10 patients per day: 45-60 minutes per appointment
- Conduct Annual Wellness Visits
- Identify preventive measures
- Create care plans for chronic conditions
- Supervising implementation of the care plans and behavioral coaching by other clinical staff
- Episodic visits as well
- Managing follow up consultations and care coordination in between visits
