Job Description
Our client is seeking an NP/PA to provide Primary health care to the geriatric population with an occasional adolescent. Interested, Please Refer to JOB ID# 52471
- Full Time- Permanent Role
- Exact schedule will be discussed during the interview
- You will be managing a panel of chronic care ambulatory patients in an office-based setting
- Support staff- Case managers and medical assistant
- Duties include treating-CHF, IVF for Dehydration/Renal Failure, COPD/Asthma, patient education, and disease management
- Must be comfortable with the ability to treat the acutely ill in an outpatient setting offering IV hydration, IV antibiotics, wound care and other services with the collaborating physician
- Ideal candidate understand the PCMH model of care
- Please have 2 years of experience
- MD license in hand
