Seeking a Coumadin Clinic Nurse Practitioner

in Medford, Oregon!

Asante Health System (Asante.org) is seeking a Nurse Practitioner to join our Coumadin Clinic team in beautiful Medford, Oregon. Working with 3 other Nurse Practitioners, the Coumadin NP will be doing anticoagulation assessments for patients taking Coumadin or Warfarin, including education and/or treatment for acute and chronic health problems along with preventative care. This is a 0.75 FTE position, working 60 hours per pay period. Shifts are 10 hours, and our providers work both in Medford, OR and in Grants Pass, OR.

OPPORTUNITY HIGHLIGHTS

Education: Graduate from a Nurse Practitioner Program.

Experience: Must have anticoagulation experience either as an RN or NP. Experience providing care for clients requiring anticoagulation therapy using Coumadin, administering Coumadin according to current evidence-based standards of practice. Experience in Coumadin therapy patient education is required.
