Job Description
Our client is seeking a Nurse Practitioner with experience in handling complex adult-geriatric patients for home visits. Please see Job ID #65854
- Full-time, permanent position
- Seeing 5 to 6 patients per day, an autonomous setting with the ability to set your own schedule!
- Candidate must have at least 2 years of experience seeing the adult-geriatric population in related settings such as Post-acute care, rehabilitation centers, assisted living facilities, and/or palliative care
- Candidates with at least 6 months of direct home health experience are welcome to apply!
- The provider will be conducting age-appropriate assessments of patients, including history taking and physical examination, Performs assessment/data collection in an ongoing systematic manner, focusing on physiologic, psychosocial, cultural, spiritual, and cognitive status, patient/family education, family involvement, and patient advocacy
- Arizona License in hand or in process
