Seeking Field Based Nurse Practitioner for Home Health
Orange County, CA
Full-time
Provide In-home patient care to complex and high-risk patients.
The program is designed to provide in-home patient care to their complex patients living with multiple chronic conditions.
60% face-to-face medical visits
20% documentation of Patients medical conditions
10% communicates with Providers for proper treatment
15% Participates in on-call
The Care Team will include, but not limited to, a nurse practitioner, nurse case managers, social worker, dietitian, pharmacy supports and palliative/hospice services
