Job Description

DirectShifts is Seeking a Hospice Nurse Practitioner to Work in Albany, NY .This is a Full Time position. My client is very motivated to speak with all interested candidates.

The selected candidate will directly prescribe care when appropriate and in collaboration with the patients' attending physician for hospice and palliative care patients to ensure compliance with the Interdisciplinary Plan of Care;

The Nurse Practitioner is responsible for providing guidance and education to the Interdisciplinary teams regarding palliative care as well as to attending physicians, team members, patients and families; and mentors students, residents and staff regarding principles and practice of hospice and palliative care.

The selected candidate will also participate in provider on call rotation.

Applicants should have the following requirements: NYS licensure and DEA required; graduation from an AMA approved program; Board Certification - Adult Nurse Practitioner (ANP); and BLS required.
