Nightingale Clinical Care, PC is looking for a full-time Nurse Practitioner Manager to join our clinical care team to support our San Bernardino, CA market.

Nightingale Clinical Care, PC is an innovative team of passionate professionals who tailor care plans for patients with complex needs, and work closely with all care team providers with the goals of cultivating a patient centered care approach, improving care consistency, and enhancing the coordination of patient services.

Nightingale Clinical Care, PC is a team of advanced clinicians led by Dr. Amy Schuerman-Gen. Driven by a goal for patient centric care and advocacy, our team utilizes our passion and skills to drive better clinical outcomes for patients by working with integrated care teams, sharing our knowledge with those teams through clinical mentorship, and providing one-on-one patient care.

As a Nightingale Nurse Practitioner Manager you will support some of the most complex patients, lead and manage a team of Nightingale Clinical Care Nurse Practitioners.

This Nurse Practitioner position is great for experienced Nurse Practitioners who want to continue to develop and grow their careers, as well as continue to grow and develop other clinical professionals.

In this role you will provide clinical leadership, oversight, coaching, supervision and onboarding of Nightingale Clinical Care Nurse Practitioners.

You will participate in population health management and overall financial viability of the program. This role functions autonomously, yet in partnership with Operational and clinical leadership on budgeting, quality initiatives, outcomes and performance measurements.


Your team of Nurse Practitioners will work closely with DaVita’s Integrated Care team to provide superior care for our dialysis patient population.

Nightingale Clinical Care, PC seeks Nurse Practitioners who practice medicine with a focus on patient care, and who truly desire to help patients lead their best life. We want our clinicians to take the time needed to address the patient’s needs and concerns.

Qualifications
  • 4-6 years of NP Experience; experience in population health, Primary/family care, and/or nephrology a plus
  • Masters degree in Nursing
  • At least 1 year experience in supervision or leadership (did not have to be a NP at time of supervision or leadership)
  • Excellent verbal and written communication skills
  • Demonstrated proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation regarding the needs and care of chronic disease
  • Deals with confidential information and/or issues using discretion and judgment
  • Intermediate computer skills and proficiency in MS Word, Excel, and Outlook
  • Excellent organizational skills and attention to detail
