Job Description

Optimum Permanent Placement is working with a well-established client in Georgia seeking Nurse Practitioners to join their team.

This position provides home-based palliative care for patients in their homes. Be apart of an after-hours care team support system to prevent unnecessary or avoidable hospitalizations and ED visits. The ideal candidate will have family practice and geriatric experience.

Opportunity Details:

-Shift info: Day shifts, 8a-5p
-No call shifts required
-Daily volume: 4 visits on travel days and 6 patients on telephonic days
-EMR: Welkin
-Requirements: Board-certified
-Competitive compensation
