Maricopa County, AZ - PMHNP - Home Care - To $150K DOE + Benefits
-Join interdisciplinary care team of home-based psych & medical care providers
-Provide care to patients facing serious illness (palliative care)
-See 1-4 patients/day at their homes
-Visit patients at their home
-Agency will reimburse miles or other travel costs
-60-90 minutes for initial evaluations + 45 minutes for follow ups
-30% homebased visits, 30%, tele-health consults, 30% remote treatment team + 10% team education
-Full Benefits, PTO/Holidays/CME
-To $150K DOE
