Job Description
Due to our continuous growth we are looking for a full-time APRN to manage behavioral health conditions and coordinate health care for the patient in accordance with State and Federal rules and regulations and nursing standards of care. Shoreline Wellness Center, LLC provides ongoing collaboration with a board certified child psychiatrist and a board certified adult psychiatrist, nurse practitioners and other mental health professionals.
Shoreline Wellness Center, LLC is a multi-specialty, group, behavioral health practice located in West Haven, CT. Shoreline Wellness Center, LLC offers individual, group, couples, and family therapy as well as medication management services for children, adolescents, adults, and the elderly.

Ideal Candidate: APRN who can provide services for a minimum of 20 hours per week. Professional, good judgement, excellent writing skills, and reliability are a must. Part-time or Full-time options available.
Responsibilities: The Behavioral Health APRN oversees daily management and coordination of behavioral health needs for patients. As a clinician, the Behavioral Health APRN provides an assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plans, and evaluation of patients status. As an educator, the APRN acts as a clinical expert, teacher and resource to patients, families, and practice staff. The APRN is also responsible for a variety of administrative duties related to efficient and effective communications between members of the health care team.
Requirements: MSN required. Graduate of an accredited school of nursing. Graduate of an accredited Nurse Practitioner program.
Required Licenses or Certifications: Active CT APRN license and Nurse Practitioner certification.
