Group has five locations in Upstate NY about 2.5 hours from Montreal, Canada.
ABOUT THE GROUP:
Group began in 1975 and now is an FQHC with 5 locations. They added their 5th location in January 2020. They provide an array of services: Primary and Pediatric care, Optometry, Dental care, Physical Therapy, Health Home Care Coordination, and Substance Abuse Support.
ABOUT THE POSITION:
Our team is currently seeking a Psychiatric Nurse Practitioner to provide psychiatric evaluations, diagnosis with recommendations for treatment, prescribe medications as conditions indicate, and conduct follow-ups. The successful candidate will be part of a comprehensive, multidisciplinary healthcare team made up of licensed clinicians, LCSW's, nurses and case managers, and will work with the team to provide quality care to the individuals served.
