We are a multi-specialty Healthcare network of Clinicians, Specialists, and Nurse Practitioners, that provide medical services to residents in senior retirement communities, assisted, independent living, group homes, and memory care communities via home health and tele-health.
With fast growth in telehealth and numerous specialties, we are now entering an exciting growth phase and are looking to continuously add Nurse Practitioners/Physician Assistants for numerous specialties. In this role you will be closely guided with a mentor and 360 degree support by our back office.
We are looking to provide all our clinicians with great growth opportunities and additionally, tflexible time schedules for our candidates in both housecall and telehealth settings.

Essential Functions
|

|
* Diagnose and treat patients in private homes or assisted living facilities, including House Call services specializing in Mental Health via “in-person” House Call and/or telemedicine visits.
* Responsible for helping patients cope with psychiatric disorders, provide treatment for disorders, including ADHD, mood disorders, and anxiety (but not limited to)
* Evaluate patients before and during their care to monitor their progress and administer medications as needed.
Job Requirements:
* AZ License must be current and in good standing
* One to two years’ experience as a Nurse Practitioner / experienced candidates welcome for leadership (mentoring) functions
* Must have current DEA and License (AZ)
