Job Description

What you'll do:

  • Overseeing day-to-day clinic operations and participant programs
  • Responsible for operating and staffing a high-performing, well-equipped, and safe clinic by ensuring workflow efficiency and clarity.
  • Ensuring protocols, policies, and procedures are well-understood and consistently adhered to by staff.
  • Supervise all the Nurse Case Managers, the Clinic Supervisor, the Care Team Assistant, the team of Nightly Navigators, and provide leadership and guidance to the entire clinic staff, including performance management, training, support, evaluating, and coaching.
  • Collaborates effectively with all stakeholders, including their market’s leadership team, other members of the care team, Home Services, Complex Care Team, outside Specialists, families, Welbe Central functions, and as needed with other leadership teams.
  • Analyzes data with attention to trends, strengths, weaknesses, and opportunities.
  • Perform root cause analysis, evaluate care delivery processes, and based on their findings, make recommendations to the Clinical Services Director on changes to strategies, workflows, and policy to continuously improve clinic operations and lead implementation of agreed-upon improvements.
  • Hiring, training, mentoring and supervising a team of RN’s, LVNs and Medical Assistants (MA).
  • Coordinating caregiving and skilled home health needs with the participant and/ or family and Interdisciplinary Team.
  • Reviewing utilization metrics to measure the effectiveness of participant care while supporting fiscal goals.
  • In collaboration with PACE Physician and Clinical Services Director, will actively participate and deploy strategies for ER Care prevention with RN, LVNs, and MAs.
  • Working with the PACE Physician, Clinical Services Director and other members of the IDT to manage smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.).
  • In collaboration with PACE Physician and Clinical Services Director, ensuring all orders as carried out as directed.
  • In coordination with the Marketing Team, supporting enrollment of prospective participants into the program.
  • Maintain a clean, safe, and audit-ready clinic environment for participants and staff.
  • Other duties as assigned.

Your qualifications:

  • Bachelor's degree in Nursing or Health-Care related field Required
  • Minimum of two years' experience in a clinical setting with a frail or elderly population preferred
  • Minimum of two years’ experience supervising an ambulatory clinic operation including RN, LVN and MA staff or equivalent experience Required
  • Highly motivated, self-directed, able to execute tasks in a quickly changing environment
  • Bilingual English/Spanish preferred
