The Opportunity - Lead Patient Care Coordinator
-Track patient cases from admission to discharge
-Ability to identify patients who are likely to suffer adverse health consequences upon discharge, needing coordination of support services and follow-up care based on diagnosis and level of acuity
- Ability to develop individualized care plan for patients needing patient care planning support
-Establishes and maintains effective communication and good working relationships with all coworkers
-Minimum 5 years of experience in an acute care setting (Hospital Experience Required)
-Able to apply National Patient Safety goals to care and documentation
