Clinical Care Coordinator
Job Description:
The Registered Professional Clinical Care Coordinator (C3) assumes the responsibility and accountability for collaboratively managing the progression of patient's care with the physician and health care team utilizing evidence based best practice guidelines and standardized nationally recognized criteria.
Required skills & experience:
- RN, ASN or BSN welcome to apply, BSN preferred.
- Three (3) years experience in an acute care setting
- Working knowledge of Care Management, UM desired
- Demonstrates excellent communication skills that will promote team building and care coordination, and role models leadership through appropriate use of clinical resources and management of patient flow across the continuum
What you need to know:
- The Clinical Care Coordinator engages the patient and care team in the joint management of key patient outcomes across the continuum employing acquired utilization management and discharge planning skills.
- The C3 serves as a resource and expert to the staff and provides on going education to staff, patients, and families related to patient flow.
