Job Description
Position: RN Clinical Coordinator
Location: Baltimore, MD
Shift: Monday – Friday, 8:30am to 5pm
Job Information
- Coordinate health care services for patients.
- Ensure quality care is delivered in an efficient and effective manner.
- Ensure customer service standards are continuously demonstrated.
- Engaging patients and their care givers in understanding and setting self-management plans in a culturally and linguistically appropriate manner.
- Facilitates and coordinates services to develop patient-centered individualized integrated self-management plans, patient care plans, including self-management and outcomes goals.
- Collaborate with various health care providers across the care continuum to ensure that patients are effectively managed and that health care needs are met.
Requirements
- Current, active and unrestricted Maryland RN licensure
- Minimum of three to five years of recent clinical experience in acute medical/surgical care;
- 3 to 4 years of ambulatory care setting experience preferred; 2 years or more of progressively responsible management experience;
- BSN preferred from an accredited college/university.
