Overview
POSITION SUMMARY: The Nurse Navigator Digital Health serves as a virtual patient liaison to help patients and family members navigate the complex healthcare system while being virtually managed in their home environment. The nurse will work from the Peoria Office. Fosters and maintains positive working relationships among staff, physicians, and medical facility personnel to ensure growth of services and programs offered. Works toward seamless care across the continuum by utilizing digital tools and technology to connect remote the multidisciplinary care team to the patient in their home. The Nurse Navigator will establish early contact with the patients using digital technology to identify and eliminate barriers to timely access and treatment. Assess clinical, emotional, spiritual, psychosocial, financial and other patient needs. The Navigator will be responsible to monitor and report on outcome measures, schedule tests, procedures, appointments and treatments as well as contribute to the plan of care. They will also direct patients and families to available community resources and supportive services. Serves as a clinical resource in reinforcing education with patients & families, in addition to providing input on educational materials. Requires ability to regularly and timely travel to various sites.
Qualifications
REQUIRED QUALIFICATIONS: Education: BSN within 6 years of hire date Licenses/Certifications: RN-Registered Nurse BLS CPR Experience: 1 year of RN experience or completion of an RN Residency program Other skills/knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software Strong analytical and problem solving skills, with the ability to be detail oriented.
