Registered Nurse: Care Partner Program- Multiple Sclerosis

Our partner is developing a Care Partner program to assist providers with caring for people living with multiple sclerosis (MS). Most of what a person living with MS experiences happens outside of the office visit. We hope to lessen the burden of managing the disease both for the person living with it as well as those providing care. By doing this, we hope to improve outcomes for all involved. We are looking for our pioneer Manager of the Care Partner Program to be the lead in developing this program. It is a balance of clinical knowledge, organizational skills, business acumen, employee engagement, and compassion.

Responsibilities Registered Nurse: Care Partner Program

Manage relapse screening protocol

Contact patients when a new or worsening symptom appears, or when the patient reaches out regarding a suspected relapse

Checks back in at 24 hours, and guides through the relapse test kit and screening questions

Escalates if necessary, to the patient’s neurologist or appropriate emergency care service

Assess and triage symptoms

Guide patients through the process of reporting symptoms

Input patient’s structured and unstructured symptomology data

Communicate with HCP when patients experience actionable symptoms

Manage side effects of DMT medication

Manage patients’ medications and communications with their pharmacy

Monitor adherence and adjust drug courses accordingly

Reach out to the patient when adherence falls below threshold

Provide encouraging messages to patients after injection cycles

Find and record patient on-boarding information and coordinate on-boarding

Interface between EHR and client

Call the patient to on-board him/her to the MS app

Customize app/service features to patient needs

Provide companionship to our clients with emotional support and motivation

Check in on patients proactively, based on their preferred method and frequency of contact

Requirements: Registered Nurse: Care Partner Program

Direct patient care in clinic with people living with MS

5 years clinical experience preferred

Worked with inject-able medications for chronic diseases

Home healthcare experience

Teaching/training experience recommended

Tele-Medicine experience a plus

Education Requirements:

Graduation from an accredited nursing program

Current nursing license in good standing

Bachelor's degree required, MPH a plus

Qualifications:

Strong organizational skills

Written and verbal communication skills

Can readily assimilate new information

Must be a US citizen or have an active green card
