Schedule- This is a 30 hour a week role. Schedule Expectations: 9 hour shifts: 9-6 pm or 10-7 pm Mon-Friday. Mandatory Every Other Weekend: 9-5 PM on Saturday. 9-4 PM on Sunday

With the Health Corner roles, the aim is to provide a safe, comfortable place for people to ask questions they might hesitate to ask in more formal care settings, in the hope that they can make health-related decisions with more confidence, and better care for their own and their family's health. Here, customers can meet with a Health Advisor ' a qualified RN who not only can answer questions about medications but also sit down in a more private format to help patients and their caregivers understand and manage ongoing health concerns, introduce them to the latest in digital health solutions, assist them in making healthy lifestyle choices and more.

Responsibilities:

' Oversee Health Corner activities and provide both in store and online diagnostic devices and solutions, and assist with proper use of devices to all customers

' Performs gap closure activity as outlined, and escalates issues as needed

' Effectively communicates with agency staff, other Care Facility staff, patients, and family/caregivers

' Manages Time effectively to deliver accurate and appropriate patient care

' Manages staff and/or leads a team as required, handle escalations related to Care Gap Closure program and other Health Corner activities and priorities

' Maintains Confidentiality of patient and proprietary information

' Maintains a professional image

' Participates and attends required in services

Basic Qualifications:

' Graduate (at LEAST an Associate's Degree, Bachelor's Degree preferred) of an accredited school of nursing with valid license in state(s) of practice

' Certification in a specialty area through a nationally accredited program (i.e., OCN, CRNI, CCM, RNC, CCRN, etc.)

' At least two years of experience providing nursing care. Self-directed with excellent time management skills and able to work independently

' Flexibility and the ability to work effectively within a multidisciplinary team

' Evidence of basic leadership skill

' Current CPR certification

' Ability to communicate effectively (verbal and written) with team members, patients, caregivers, physicians and referral sources Willingness to work a flexible schedule, including evening and weekend hours

' Comfortable with digital technologies such as website forms, mobile applications, PC/tablet systems, and in-store systems and connected healthcare devices

' Ability to get to the job site

Preferred Qualifications:

' Two years of experience working in high touch retail environment; prior knowledge of Clover Health Care System a plus

' Experience in assisting customers to resolve complex issues
